Abstract: In this report we present the results of our observations of 2 patients in the New York City area with ulcers on the plantar surface. Our purpose in describing these cases is to emphasize the need to consiuder hansens diase in the diagnosis of ulcers of the foot. We have been able to heal theserefractory ulcers by the use of sodium sulfoxone (Diasone sodium), the specific for hansens disease. A critical evaluation of the newr technics for confirmation of this diagnosis is presented. The awareness of the existence of hansens disease helps to differentiate it from the more common causes of leg and foot ulcers in peripheral vascular diseases. Thus there is a need to evaluate all lower extremity ulcers with specifc diagnostic tests, such as blood sugar for diabetes cryoglobulins for cryoglobulinemia ulcer biopsy for epithelioma and a complete blood count bone marrow biopsy and coombs test for sickle cell anemia cooleys anemia and hemolytic anemia. A careful history and accurate physical examination can usually differentiate a thromboangiitis obliterans ulcer from that of peripheral arteriosclerosis obliterans. Similarly the ulcer of varicose veins is differentiated from that of diabetic neuritis by physival evaluation and the postphlebilitic ulcer is differenbtiated from that of choronic lymphedema by lymphangiography. The following are 2 typical cases of foot ulcers due to hansens disease found in the New York City area. (AU). Abstract: Plantar ulcers in leprosy is not specific of the disease, it instead represents one of the neuropathic disorders. However, plantar ulcer is one of the commonest lesion in leprosy (Cochrane 1967) and no singular method of its treatment is known to provide the best results. Abstract: Le repos et la correction éventuelle des déformitions (transplantations tendineuses - résection des saillies osseuses) assurent la cicatrisation du plus grand nombre des maux perforants plantaires lépreux lorsqu`ils ne sont pas compliqués d`ostéites ou d´ostéo-arthrites sous-jacentes. Les plasties cutanées souvent indiquées et la résection des articulations surinfectées parfois nécessaires n`ont qu`un interêt temporaire : dans tous les cas, les récidives sont fréquentes. La neurolyse du nerf tibial postérieur et des nerfs plantaires au canal tarsien assure la guérison et prévient la récidive dans 85 per cents des cas (AU). Abstract: Se trata de la evaluación del tratamiento quirúrgico y ortopédico realizado en San Pablo (Perú), realizándose 140 cirugías de exéresis a 88 pacientes de Hansen, la mayoría de ellos negativos y curados, pero portadores de mal perforante plantar, consideramos por ello, inválidos e incurables. Se ha conseguido, en nuestro medio, reintegrar a la sociedade a estos antiguos pacientes, muchos de los cuales tras beneficiarce de la cirurgía y ortopedia, caminan depués de muchos años en absoluta invalidez e inmovilidad en sus camas. Esta cirugía se ha realizado en este Centro de Salud como parte complementaria del Programa Control de Lepra, sin interferir con alas actividades de atención primaria de salud. Creemos muy importante para la prevención primaria, la educación del cuidado del pie y su protección mediante el uso de zapatos especiales para pie anestésico. Considerándose importante que la ortopedia sea sencilla y funcional, adaptada a los recursos y condiciones del lugar, de acuerdo a las posibilidades del país. (AU).
